If you are located in Alaska, Montana, Wyoming, Washington, Idaho, Oregon, Utah, North Dakota or South Dakota, you will have the flexibility to telecommute* as you take on some tough challenges.


Primary Responsibilities:

  • Schedule regular follow-up for fee-for-service (FFS) Clients with appropriate clinical and non-clinical staff based on the Alaska's specific FFS behavioral healthcare needs and as described in the plan of care.
  • Review assessments and assessed acuity level, diagnosis, and medical history and update the FFS Client's information and plan of care at subsequent contacts with the FFS Client.
  • Engage with providers and behavioral health treatment teams in discharge planning and timely, successful transition to the community of Medicaid recipients.
  • Provide disease and intensive care management services for Alaska Medicaid beneficiaries.
  • Conduct telephonic and/or face-to-face independent assessment of the needs of Recipients determined to be eligible for the State Plan benefits to ensure Recipient's needs are assessed and documented and to establish a plan of care. Conduct the independent assessments with the use of standardized assessment tools.
  • Develop person-centered service plans with Recipients, and/or their authorized representatives.
  • Evaluate members' ability to independently manage self and locate alternative resources when limitations are identified through a psychosocial assessment.
  • Assists recipients in accessing verified community, government, and/or client resources.
  • Facilitate member care transition through the healthcare continuum and refers treatment plans/plan of care to clinical reviewers as required
  • Participate in Quality Control and Process Improvement (QC/PI) activities.
  • Other duties as assigned.
