The overall goal of the Peer Specialist program is to help promote recovery within an acute-care-psychiatric setting by engaging with clients and in assisting in the establishment of secure community connections of all kinds. The Peer Support Specialist will work alongside individuals during and after a psychiatric hospitalization to encourage wellness and independence. The Peer Support Specialist will advocate for those treatment options the member identifies as needed and helpful to their recovery journey. The Peer Support Specialist helps to foster hope and serves as a role model for persons who are transitioning back into the community.  Common activities of the Peer Support Specialist include assistance with making a connection with an outpatient provider(s), in increasing natural support networks; effective utilization of community resources; support in receiving vocational rehabilitation services; involvement in meaningful community activities; support and encouragement for increased self-advocacy; and other individually identified activities that enhance self-esteem, hope, and empowerment. 


 


Primary Responsibilities: 


  • Engage individuals to form trust-based relationships that foster hope for positive change and personal empowerment
  • Offer a positive supportive relationship that helps promote hope, choice, wellness and recovery for participants
  • Provide assistance in identifying and linking to, community-based services and supports including outpatient providers, i.e., (primary care, mental health and substance use disorder treatment, housing, financial benefits, etc.)  Peer support services will be offered in a mutually agreed upon location in the community or in the consumer's place of residence
  • Provide information and support related to the experience of being hospitalized and participating in substance use disorder treatment
  • Provide one-to-one and group support during hospitalization.   Continue to provide short-term-support services following hospital discharge
  • Assistance with developing Wellness Recovery Action Plans (WRAP)
  • Assistance with coordination of transportation to appointments using mass transit
  • Help with identifying and/or re-engaging with family, friends, and other natural supports
  • Provide education and resources that support illness self-management, self-determination, self-advocacy, and shared-health-care decision making
  • This position requires that the selected candidate be able to work a flexible schedule including an occasional evening and weekends
  • Other duties as required and assigned
