GENERAL STATEMENT OF RESPONSIBILITIES:

The Case Manager is responsible for the management of youth treatment services and youth case management services, works with the Court and may attend Court hearings. This position works directly with the youth, the family and the County or State workers utilizing a cognitive behavioral and strength based approach in the assessment, planning and treatment stages and task work. Responsibilities include, treatment planning, completing timely reports, coordinating comprehensive treatment services, conducting cognitive behavioral individual and group therapy utilizing the agency model, and facilitating psycho-& shy; educational and support groups. Work is with the youth, family, treatment team, youth workers and adjunct workers to achieve identified goals.

Statement of Duties

  • Ensures that the policies & procedures and program methodologies are practiced as defined by program standard operational procedures, program manuals and training sessions.
  • Monitors all services to ensure that services have been delivered as arranged and that the youth's needs are being met. Ensures that program and contract requirements are met. Ensures that good casework is provided by monitoring case decisions, reviewing records, conducting audits and consulting with the Clinical Service Manager and the treatment.
  • Maintains an effective system for tracking case assignments, caseloads, reporting, and any other service event in order to ensure compliance with licensing and program requirements.
  • Facilitates and maintains documentation of at least one hour of individual meetings per week with each assigned youth. Individual meetings are to be a minimum of 30 minutes in length and provide an opportunity to offer ongoing treatment to the youth, discuss the youth's progress and challenges and keep the youth informed of any legal proceedings.
  • Responsible for overall case management insuring that all treatment needs are met for assigned youth. Case management includes linking and coordinating the child and family with all services to various providers.
  • Responsible for all case coordination tasks insuring that all members of the Treatment team CMO Case Manager, family, and other adjunct workers are kept abreast of assigned youths' progress.
  • Responsible for the development of the written Court Reports and Treatment Plans. The treatment plan must be individualized and consistent with the youth's treatment needs. Inputs all required documentation into JAIS and agency reporting systems within required timeframes.
  • Responsible for creating and maintaining all case files. Responsible for insuring that all required written paper and electronic documentation is submitted to the required bodies within the established timeframes, this includes but is not limited to Treatment Plans, Progress Updates, Court Reports, Incident Reports, etc.
  • Facilitates a minimum of one Case Conference per month that includes the youth, participating family members, CMO/State workers and relevant SJJS team members. Completes required documentation of the Case Conference.
  • Gathers all pertinent information towards the development of the youth's social history for the Initial Treatment Plan using the Initial Interview Guide and court case records as a basis.

  • Obtains pre-approval for Reintegration Visits, coordinates all visitations and maintains documentation of all visits in the case files.
  • Keeps the CMO/State worker continuously informed of youth progress, initiating bi-weekly telephone or in-person contact and immediately informing the CMO/State worker of any significant events.
  • Keeps the Supervisors informed of case status, special client needs, and any unusual incidents.
  • Actively works towards the initial engagement of the family (unless contraindicated) by telephoning the family/significant persons the day of the youth's arrival and mailing the Welcome Pack on the first day of the youth's arrival. Participates in home visits when necessary to promote family involvement.
  • Participates in Family Forums by planning and implementing sessions that serve to provide programmatic information and relevant material to families affiliated with residents of SJJS Treatment Programs.
  • Supports Youth Workers by providing ongoing training regarding roles and responsibilities to insure structured implementation of the treatment plan and program. In conjunction with the supervisory staff, provides leadership and direction to Youth Workers, working as an integral part of the Treatment Team.
  • Monitors Youth Worker interactions with youth and receives daily reports from Youth Workers and Teachers on all youth progress and challenges.
  • Meets with the youth on the day of the youth's admission to provide an extensive program orientation to the youth. Minimally, these orientation meetings are one hour in length.
  • Participates in 24 hours/7 days on call availability for client/program needs.
  • Facilitates and observes problem solving sessions as assigned.
  • Coordinates weekly Interdisciplinary Team Meetings for assigned pod.
  • Coordinates drug testing for the youth, as needed.
  • Coordinates psychiatric services by completing timely referrals for psychiatric evaluations and medication reviews and communicating scheduling and related information to the resident, parent/guardian and all members of the Treatment Team.
  • Works in conjunction with the Psychiatrist to provide ongoing psychiatric support.
  • Monitors youth medications, medical needs and medical treatment on a weekly basis.
  • Delivers all services in accordance with SJJS Clinical Philosophy.
  • Maintains case supervision until case closure including aftercare services as determined by the Courts or contractual obligations.
  • Represents program in agency-wide administrative committees and quality teams as assigned.
  • Practices a team oriented approach to program development, and promotes group input into case planning, treatment, and service delivery.
  • Participates in Agency training programs; learns and delivers the Agencys program design and methodologies.
  • Participates in Family Forums by planning and implementing sessions that serve to provide programmatic information and relevant material to families affiliated with residents of SJJS Treatment Programs.
  • Responsible for adhering to the agency's standards of professional and ethical conduct and must perform the assigned job tasks in a professional manner and to manifest behavior consistent with the values of compassion, justice and respect for the dignity of others.
  • Responsible for adhering to SJJS Policies, Spectrum Administrative Operations Manual, Spectrum Personnel Policies, all State and County rules and regulations regarding licensing, juvenile justice and child welfare and all other information dictating service provision.
  • Completes all other assignments and assigned duties as given by SJJS Clinical Supervisory Staff and Program Administrative Staff.
  • Employees in this position are expected to have working knowledge of and remain current on the following service standards:

Qualifications and Skills

Master's degree in Social Work, Counseling or Psychology. It is highly desirable that this person has group work experience and at least two years of institution experience.

The person in this position should be able to meet State Licensing Standards, and should be able to be registered as a Social Worker, Counselor or Licensed Psychologist. This requires a telephone in the home and computer competency.

Must meet the following requirements: Possess a valid State of Michigan Drivers License. Must complete and return a record of no findings on the FIA Central Registry Clearance, a criminal history background check free of felony convictions, a physical examination which indicates satisfactory health status free from communicable diseases including TB and a physical ability to perform the functions of this job description. Must have reliable transportation.

Experience in direct treatment services including residential treatment, casework, group work and behavior modification process. Experience in Cognitive Behavioral Therapy preferred.

The selected candidate must successfully complete a comprehensive background check including medical clearance (TB test, drug screen, physical), DHHS clearance and MVR clearance.
