Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 16 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. InterCommunity's Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

We are seeking a compassionate, caring and professional Assessment/Mobile Crisis Clinician. (bi-lingual preferred but not required)

Assessment/Mobile Crisis Clinicians help individuals referred to Intercommunity services by providing high-quality level of care determinations and comprehensive behavioral health assessments using evidence-based clinical tools and sound clinical judgment. They are talented clinical social workers or psychotherapists with a passion for and a commitment to working with adults.

The clinician is responsible for completing assessments and emergency crisis assessments and safety plans, in hospitals, at home, and in the community. Services include phone crisis screening and triage; in-person crisis evaluation and crisis intervention services; and linkage to an appropriate level of follow-up .The clinician will provide emergency crisis intervention services to adults as needed in the following municipalities: East Hartford, Glastonbury, Marlborough, Newington, Rocky Hill, Wethersfield who are acutely suicidal, homicidal, or gravely disabled.

Essential Duties & Responsibilities:

  • Provides trauma informed clinical services in form of triage, crisis intervention, assessment, individual, group and/or family therapy, screening, treatment planning and referral within a multidisciplinary team and departmental coverage for staff absences
  • Accountable for independently providing trauma informed clinical services
  • Completes all required documentation related to client treatment in accordance with established procedures
  • Provides coordination of care and referral with other community agencies i.e. hospitals, emergency rooms, jails, homeless shelters, outpatient programs, etc.) and consults and collaborates with other behavioral/ medical health providers.
  • Meets productivity standard for the position and role
  • Attendance and participation in participation in all required departmental staff meetings, all provider meetings, supervision ( including peer) meetings including but not limited to systems feedback, problem solving, and support for development protocols which enhance service delivery.
  • Participation in trainings to bring new service delivery options to clinical care ( ie EMDR, ART etc)
  • new models of treatment , evidence based practices)
  • Internal capacity building ( i.e. staff embedded in schools, community outreach and education)
  • Performs related duties as required

Specific Responsibilities within the MCT

Provide immediate phone screening and prompt mobile response,

Provide crisis assessment and short-term intervention for adults focused on safety planning and crisis stabilization

Provide assessment, and treatment services to victims and witnesses of crime and other potentially traumatic events

Obtain immediate case consultation and supervision, when needed for support, enhanced decision making and safety in high risk situations.

Prioritize personal safety through compliance with safety policies and procedures

Coordinate with Director of Assessment Center and Mobile Crisis to promote successful transfers between MCT and other programs

Maintain positive collaborative relationships with key community partners i.e. 211, hospital emergency departments and in-patient units; other higher level of care programs; police, , victim assistance programs, etc.

Specific Responsibilities within the Assessment Center

Complete a clinical assessment of adults mental health and/or addiction recovery needs and gather necessary data to support a recommendation for services

Meet with the individual to obtain a comprehensive history and detailed information about the adults concerns/needs

Obtain necessary clinical data to justify recommended support services through observation and collaboration with involved resources

Understand IC systems of care and make appropriate recommendations for support services

Complete thorough and detailed Individualized Treatment plan

Submit paperwork within program guidelines

Education &/Or Experience:

Master's Degree in Behavioral Health discipline from an Accredited University advanced Licensure in the State of Connecticut as a Clinical Social Worker, Marriage and Family Therapist, Psychologist, ( LCSW, LPC, LMFT) with 2 years experience in recovery orientated system of care in a behavioral health setting.
