Cedar Hills Hospital and Outpatient Services - located just 15 minutes from downtown Portland - is a 98-bed, free-standing psychiatric hospital and outpatient center specializing in mental health and substance use disorder treatment. We provide a wide range of services and programs that offer evidence-based treatment proven to have positive outcomes for our patients. We are seeking an energetic team player who can accurately track, transmit forms and records, provide support to the Health Information Management Department. Processes and completes requests for release of information. Supports the facility by responding to requests for patient information and confirms authorizations for such requests. Key emphasis is placed on following all requirements of State and Federal regulations related to PH&S, HIPAA compliance policies and procedures. Responsible for any other duties assigned by the Program Manager of Health Information Management to interface with internal and external personnel and clients.

- Consult with management to ensure that all patient records are maintained within hospital standards.

- Research and respond to requests for patient information, other routine and emergent.

-Follow through with the preparation and/or revisions to medical record, forms, ensuring compliance with all requirements of JCAHO, applicable state and federal laws, by laws and rules/regulations.

-Investigate requests for information, protect patient confidentiality, demonstrate ability to verify information, and update the data base and log communication actions and resolution.

- Determine location of requested records and retrieve them.

-Maintain patient charts by entering patient information into computer files, and also in paper records. Organize, manage, and sort paperwork (including patients' charts). Continue to enter data as patients are subjected to diagnostic tests and receive treatments

Job Requirements

Education: High school graduate or equivalent combination of education and experience.

Experience: A minimum of one (1) year experience with knowledge of medical records coding and medical terminology, and an understanding of the clinical information environment.

Additional Requirements: Demonstrate a detailed knowledge of computer proficiency and word processing. Knowledge of:

- General hospital operations as well as office routines

-Laws, regulations of federal/state agencies, JCAHO,and medical staff bylaws regarding medical records

- Policies and procedures regarding the management and confidentiality of patient information

- Medical records coding and medical terminology.

ICD-10 coding experience preferred.

Training: Successful completion of Crisis Management and Service Excellence training must occur within 90 days of employment and prior to assisting in any restraining procedures. May be required to work occasional overtime and flexible hours.
