Positions in this function are responsible for working directly to support patients, families, group home caregivers, pharmacies, home health agencies, and clinicians in an administrative capacity.

To be a Marketing Liaison for 4C's Physician Group to the Post Acute Facilities served by our target market. This would include regular check-in visits with Administrative Staff in all facilities where we already have a provider placed. In addition setting up introductory meetings in additional facilities that are part of our strategic targeting for growth and expansion. Assisting the Lead Physician in gathering and presenting to existing and potential facilities our provider 'metrics' to ensure we continue to be their physician group of choice in the marketplace.

Second objective will be a PPatient Navigator for the patients coming from ourHospitalist Teams to our Post Acute facilities. Integrating with the 4C's Hospital Liaisons to ensure we capture both payer and provider driven exclusivity in transfers from acute to Post Acute to maintain provider group continuity whenever possible.

  • Marketing to Post Acute Facilities: Existing and Targeted.

  • Maintain Marketing Materials for Post Acute Division.
  • ""
  • Update Acute to Post Acute 'preferred facilities tool' for patient contact in acute.
  • ""
  • Assist with communicating with hospital Liaisons for patient transfer from acute to Post Acute.
  • ""
  • Assist Lead Physician in providing feedback on quality and metrics from facilities to our 4C's providers.
  • ""
  • Liaison with our Payer Partners in the marketplace to communicate quality and metrics to and from our providers.
  • ""
  • Facilitating Post Acute meetings and Minute taking.
  • ""
  • Liaison between 4CMG and OCN for Post Acute service line.

Required Qualifications:

  • High school education or equivalent experience
  • ""
  • Prior experience in the following areas, including but not limited to:""
      """
    • Scheduling
    • """
    • Referral and auth processing
    • """
    • Medical records request processing
    • """
    • Liaising with outside entities to coordinate clinical care and administrative tasks
    • """
    • Insurance eligibility verification
    • """
    • 3-5 years' experience in healthcare
    • ""
    ""
  • ""
  • You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role.
  • ""
  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained.
