::Post-Acute Relationship Manager strategically plans, implements and evaluates for effectiveness outreach efforts designed to improve relationships with post-acute services providers including, but not limited to, nursing homes, skilled nursing facilities, home health agencies and LTACs. Assists with development of overall post-acute care strategy. Improved relationships should be evaluated based on documented growth in patient referrals to South Texas Health System from baseline and annually thereafter. Monitors and reports, in a form and format approved by the South Texas RVP, utilization of STHS by post-acute service providers as well as referrals by STHS to post-acute providers. Monitors and reports, in a form and format approved by the South Texas RVP, the clinical performance of post-acute providers such as, but not limited to, present on admission, readmission rate (as well as reasons why), ED transfers, imaging services/lab services used at an FED location for a nursing home/SNF resident when transported there by the nursing home/SNF. Based on trends identified, and with the prior approval of the STHS leadership team, engages with these post-acute providers to share relevant data on utilization/quality and proposes plans for improvement. Develop trust-based, enduring relations with post-acute entity leadership team members, care givers/care managers and physicians working in those sites. Leads the creation and hosting of a Post-Acute Symposium and trainings for Post-Acute providers as needed. Participate as requested in marketing and system representation internally and externally as South Texas Health System. Will require day travel throughout STHS local market and possible overnight travel within Regional Market as determined by business needs. Occasional evening and weekend events/work as needed. Other duties as assigned.

Job Requirements
  1. Bachelors Degree required Masters preferred
  2. Degree preference are for Healthcare, Nursing, and Social work, Marketing, Communication, Public Relations or Management.
  3. Five (5) to seven (7) years of healthcare experience required.
  4. Bilingual preferred (Spanish)
  5. Prefer one year previous marketing experience
  6. Must be able to deal appropriately with highly sensitive and confidential material.
  7. Must have knowledge of standards and laws applicable to post-acute/referral process, planning and business development.
  8. Computer proficiency to include word processing, spreadsheet and database.
  9. Strong organizational skills to include metrics management, independent scheduling and record tracking.
  10. Requires the ability to travel by automobile in the service area on a regular basis. Frequently required to work early morning, evening and occasional weekend hours.
  11. Must demonstrate commitment and adherence to STHSs Compliance Program and Code of Conduct through compliance with all policies and procedures, the Code of Conduct, attendance at required training and immediately reporting suspected compliance issue(s) to the Compliance Officer.


  1. Licensed in the State of Texas ( RN,LMSW; LMSW)
  2. Bachelors in Healthcare or related field required Masters preferred
