The Program Manager, Care Coordination role is an opportunity for a self-driven, collaborative manager to build an integrated, cohesive solution across Optum business units and key client services. This role is critical to ensuring Optum meets and exceeds our client expectations in building a new approach to Care Coordination. The Manager will take a client- and patient-centric approach to program management, balanced with meeting Optum financial and non-financial business goals. We are looking for a proactive professional who is client savvy and can effectively execute against business objectives. This individual will report directly to the Vice President, Care Coordination, building structure to ensure seamless, consistent delivery of services and solutions.

The successful candidate must be passionate about driving improvements in performance, effective at working in a fast-paced, high energy environment and confident in their interactions with senior executives, providers, and business partners.

Primary Responsibilities:
  • Supports the Vice President, Care Coordination in strategy development, solution design and execution through documentation, tracking and coordination activities
  • Supports the combined client and Optum Care Coordination team by identifying opportunities and risks, facilitating solutions and maintaining alignment with cross-functional priorities
  • Builds trusting relationships with senior leaders, clinicians and business partners
  • Drives clear, concise lines of communication with key stakeholders across Optum and client teams to ensure effective implementation of service commitments, capturing needs and acting as an advisor and influencer with senior leaders
  • Ensures cross-project cohesion by identifying areas of dependency and collaboration, scheduling and facilitating team meetings to ensure cross-business organization and harmonization
  • Supports client relationship and program management activities, including but not limited to:
    • manages historical, current and future state Care Coordination services content, ensuring accessibility to team members;
    • creates and manages the Care Coordination project plan and scoping documents; including tasks, activities and milestones;
    • organizes status reports, identifying and escalating risks and issues when appropriate;
    • manages and tracks Care Coordination data and information requests and documentation;
    • coordinates across business units to create cohesive, client-ready business deliverables; and
    • tracks performance against contractual obligations
    • helps translate data into business intelligence
    • develops high impact client facing presentations and communications
  • Provides thoughtful input to optimize overall Care Coordination performance, advising leaders on performance management and improvement activities
  • Works with client and Optum Care Coordination team to facilitate performance improvement projects
  • Works with Care Coordination leadership to establish and track measured outcomes, criteria, standards and levels using appropriate methods
  • Supports service deployment and closely monitors performance, working with finance and operations to ensure financial viability and operational excellence
  • Identifies business unit gaps and helps to develop action plans to mitigate risks and issues
  • Collects business requirements for solution design and validate requirements with internal product and client leads
  • Provides additional support as needed: implementation PM, business analyst, solution design PM as needed to support transformation
  • Supports onboarding of new team members

Required Qualifications:
  • Undergraduate degree or equivalent experience
  • 5+ years of experience in management consulting, business development, performance improvement or program management
  • 3+ years of experience in health care
  • Strong understanding of health care economics, health plan contracting, quality, and value-based care
  • Experience in customer relationship management
  • Proficient in PowerPoint, Excel, Tableau, and Microsoft 365 tools
  • Primary residence must be located in Northern California, and able to commute to the East Bay to work onsite in the office up to 5 days per week
  • Demonstrated planning, organization, analytical and problem-solving skills
  • Strong presentation, written and verbal communication skills, including communicating with senior leadership
  • Strong strategic thinking and business acumen with the ability to align clinical strategies and recommendations with business objectives
  • Proven track record working collaboratively with internal business partners and stakeholders across a large matrixed organization
  • Ability to develop relationships with clinicians and business leadership
  • Curious, adaptable and flexible style; able to thrive in fast-paced, ambiguous, unpredictable situations
  • Self-guided, motivated and able to simultaneously manage multiple activities with little direction
  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive VoiceResponse (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained
  • You will be asked to perform this role in an office setting or other company location
Preferred Qualifications:
  • Master's degree MBA, MHA, MSN, MPH or equivalent
  • Population health management, care management or care coordination service experience
  • Experience with lean performance management systems/methods
